Overview of Innovation:
Clinical CARPS (C-CARPS) is a simple to use clinical task management system. Users have been delighted with the speed of implementation & the ability to fine tune the system to their needs
The system improves patient care outcomes & the experience of clinical staff in a very cost effective manner by saving time, optimising the deployment of staff resources & identifying areas for improvement. 
C-CARPS has been implemented with great success by the Hospital at Night team at The Alfred Hospital, Melbourne with our co-developer partners, InControl Australia. Alfred Health have cited many achievements facilitated or directly enabled by the system:
  • improved performance against patient care targets e.g. 8% increase in emergency patients admitted within 4 hours
  • increased overall productivity
  • improved staff attitude towards the overnight shift
Alfred Health won the “The Premier’s Award for Advancing Healthcare 2013”  & "The Metropolitan Health Workforce Initiative Award 2014" for their achievements, in which C-CARPS was instrumental.
C-CARPS replaces the existing method of task allocation, typically reliant on paging & phone calls between staff to clarify patient needs. In simple terms it is used as follows:
  • Nursing staff issue clinical task requests from ward PC’s or mobile devices
  • Team members carry a mobile device to track, manage & update allocated tasks
  • “Clinical Leads” have the ability to re-allocate work according to demand
  • Handovers are managed quickly & safely through the system
Task requests contain all the information required to immediately prioritise & complete the task. Geographic, nurse & patient identifiers provided by C-CARPS task requests optimise efficient task allocation, improve communication & help prevent errors
The ‘Clinical Lead’ can see all tasks in real time and redistribute them as appropriate. This allows free staff to ease the burden on others, preventing individuals from being overwhelmed & minimising delays. Clinical Leads now redistribute around 14% of the work on a nightly basis.
Data from C-CARPS is used to identify long trends & to understand the type of work carried out by specific staff at night. This has provided supporting evidence in cases of complaints or concerns from staff & enabled the implementation of organisational or procedural change to improve staff utilisation & patient care. In one instance, C-CARPS data was used to reduce the overnight drug chart re-writes average of 42 to 2.5

ROI
  • The potential exists for very high return on investment, depending on the relative efficiency of the hospital’s existing processes. A user of the original CARPS system cited recovering the original investment every four months! The non-clinical version of CARPS typically recovers initial investment within 18 months. We are awaiting analysis of data on installed versions of Clinical CARPS. We are expecting similar or even better results, due to potential cost savings within a clinical environment.

    Note: Our previous model is used successfully within 15 UK NHS trusts to mange non-clinical tasks
Current and planned activity: 
Currently ‘Clinical CARPS’, Clinical Task Management System is being used in Australia and seeking first adoption within the NHS in the UK. We are looking to NHS organisations within the West Midlands to discuss the possibility of adopting the clinical version on a trial-to-permanent basis.
 
We are seeking collaborations with West Midlands NHS trusts, clinical speciality services or an NHS region to experience our technology, understand the simplicity of installation and to witness the benefits that it can bring to managing the clinical workload, especially out of hours where resources may be limited or stretched and thus not immediately available unless the work flow is adjusted. 

Evaluation/Validation – We are seeking a trial centre to work with to gain additional evidence & validation of the benefits that our software delivers

Regional Scalability:

All of our solutions are highly scalable and an installation of CARPS software will typically lead to a future expansion of the system within that site or further installations at other sites within the region or company, usually by word of mouth promotion. Pricing for individual software licences will come down where larger systems are purchased within a single contract. Depending on the needs of the client we would expect to have a single installation up and running within 2 - 3 months.

Measures:

Quality: We expect Clinical CARPS to improve the quality of communication regarding clinical task allocation, for instance Clinical CARPS task requests can be configured for required minimum information to be mandatory. We would expect this to be measurable by a reduction in the amount of time spent on task allocation (phone calls, conversations, etc) and an increase in productivity in the undertaking of the clinical tasks themselves (delivering patient care). We would expect the enhanced analytical possibilities enabled by CARPS data capture to result in the identification of improved working processes, which should also be taken into account as measures of the system's success.
Safety: We expect patient care outcomes to improve based on the fact that Clinicians and nursing staff have more time available to deliver care as above and can re-allocate work according to demand, priority, availability and suitability. We expect to see measurable improvements against patient safety targets such as improved performance against the four hour admission target from emergencey and a reduction in errors and delays to patient care. As above, any improvements to patient safety made possible through CARPS enhanced data capture should also be counted.
Cost: Return on investment can be measured against increases in productivity on the front line and relating to time saved on performance analysis and the production of reports performed my clinical Management. We would expect complaints to be reduced and costs around the complaints process should also be factored in. A normal CARPS system would be expected to cover the cost of implementation within 18 months and we might cautiously expect even better results within the clinical environment, due to the increased value of the resources involved.
People: Feedback from users and stakeholders should give evidence of an improved attitude towards the way clinical tasks are managed compared with the preceeding system.

Adoption target:

Minimum viability would be the out-of hours clinical team at one hospital. After confirmation of the system's success, we would hope to expand use to 24/7 and to implement the system at other hospitals within the WM region.

Innovation 'Elevator Pitch':
AposTherapy can reduce elective care waiting lists and costs significantly, whilst maintaining excellent clinical outcomes and patient satisfaction. For the CCG's/ Trusts with long waiting lists it will be a game changer.
Overview of Innovation:
AposTherapy technology is not new science. AposTherapy incorporates traditional concepts used in physiotherapy and surgery and puts it all into one easy to use device. AposTherapy methodology is based on changing the forces working on the joints and increasing proprioception through controlled instability. By doing this the biomechanics of movement can be changed whilst new/forgotton neuromuscular pathways can be formed.
For example, a patient who has medial knee osteoarthritis is likely to have varus knee alignment, increasing the adductor moment on the knee - therefore increasing pain and difficulty with movement/ weightbearing. AposTherapy can reduce this adductor moment to allow soft tissue around the joint to work in a more efficient way.
Gradually through frequent short daily usage (like slippers) the body will learn better improved movement patterns/ motor pathways, so even when not using AposTherapy the improvements will be maintained. This incorporates functional rehabilitation and self-management techniques.
 
Compliance to treatment is exceptionally high due to the immediate pain-relieving effects of the device.
The AposTherapy device is a footworn pair of shoes, with convex components in specific positions - depending on the patient’s mobility, irritability and severity of symptoms - so it is completely individual to each patient.
 
Patients are screened over the phone for suitability, and then fully assessed by a physiotherapist using internationally recognised PROMS, spatio-temporal gait analysis and traditional physiotherapy assessment techniques. If the treatment is a suitable option for the patient then they will enter into an initial year plan, with 5-6 follow ups spread across 12 months to work towards their goals. At each follow up all outcomes and the device are reassessed and progressed as required.
 
During follow up years we expect patients to receive follow ups for maintenance / continued improvements.
 
AposTherapy is both a Product and Service - as the device is only fully effective through attendance to follow ups and following a treatment programme. Throughout treatment, patients will receive ongoing advice on pain management and graded exercise.
 
Intended patient group size is at least 50% of patients who meet criteria for hip/ knee replacement surgery.

It also means waiting lists for surgeries have a better chance of meeting 18 week pathways.
-Improves skills of the physiotherapist work force by teaching them specialist gait analysis skills, and the implementation of a new treatment.
-Reduced GP/ healthcare professional visits
-Reduces analgesics consumption/ prescription
-Help with the fight against increasing antibiotic intolerance by reducing the number of surgeries
- Reduce obesity by reducing lower limb joint pain in this population to allow them to be more active

Innovation 'Elevator Pitch':
The TUC Safety Valve eliminates risk associated in treating patients requiring TUC by preventing inadvertent balloon inflation in the urethra thus preventing traumatic injuries which prove costly to both patient safety and health system expenditure.
Overview of Innovation:
Urinary catheter related injuries typically occur in patients when the catheter’s anchoring balloon is inadvertently inflated in the urethra instead of correct inflation in the urinary bladder. Approximately 1.3%-7% of patients receiving an indwelling catheter will sustain iatrogenic urethral trauma during the insertion process through inadvertent balloon inflation whilst misplaced in the urethra. Many more patients outside of the hospital setting (ie. community care) rely on TUC as part of their daily lives, thus the scale of the issue at large is under reported. Injuries are more common in vulnerable patient groups such as spinal cord injury patients, pregnant females with distorted urethral anatomy and elderly men dependent on long-term urinary catheters or supra-pubic catheters.
 
In 2016, a prospective study in two hospitals from Republic of Ireland over 6 months highlighted that 37 urethral injuries occurred across the two hospitals during the placement of 2,750 catheters resulting in a healthcare cost of €335,377 exclusive of long-term complications, outpatient care and medico-legal costs. There were an additional 330 bed days and 17 ICU days required as a direct result of these injuries. This resulted in a cost in excess of €122 per catheter placed in these hospitals to cover the costs of iatrogenic injuries caused during the procedures. In 2019, a follow up study from the 37 patients who received urethral injuries highlighted 2 patients died as a result of urethral trauma related to TUC.

The patent protected TUC Safety Valve, is a novel, innovative, clinically proven technology preventing the risk of inadvertent inflation of the Foley catheter retention balloon in a patient's urethra instead of the bladder, as intended. It uses a safety pressure relief valve to indicate misplacement of the retention balloon during catheterisation, eliminating urethral damage. Once the valve has “popped” it will automatically deactivate once the user ceases depressing the syringe plunger so that it functions effectively throughout the procedure. The flow restrictor prevents rapid inflation of the retention balloon (an act which has the potential to allow a portion of the fluid to bypass the pressure valve and cause partial inflation of the balloon even when mispositioned) in the urethra.

Urinary catheterisation yields significant volume of iatrogenic injuries with serious short & long-term patient complications that have substantial financial, resource, and medico-legal implications. Such injuries lead to an increased burden on resources, inpatient/ICU beds and workforce in the NHS.

Up to 25% of all hospitalised patients undergo routine Trans-Urethral Catheterisation (TUC) during their stay. Many more patients outside of the hospital setting (ie. community care) rely on TUC as part of their daily lives, thus the scale of the issue at large is under reported. TUC is more challenging in male patients due to the tortuous anatomy of the male urethra and due to prostatic hyperplasia however this risk is not limited to male patients nor solely adult patients.
Another cost measurement to consider is medico-legal claims against health organisations. In a 2011 British Journal Urology International publication, between 1995 to 2009 in the NHS 493 urology related claims were closed with indemnity payment with an average of 35 successful claims per year. In all, £20,508,686 was paid out, averaging £41,599 per claim.

Assuming a conservative 0.55% incidence rate (lower than the studied incidence rate), using the example of an 1,800 bed capacity NHS Trust:
  • 21,600 catheterisations per annum @ £10 per catheter kit = £216,000
  • 118 urethral traumas per annum (21,600 catheterisations * 0.55% incidence rate)
  • (1053 additional bed days, 54.5 additional ICU bed days)
  • £948,542 in complication costs + £216,000 total catheter spend = £1,164,542
  • 21,600 TUC Safety Valve @ £15 = £324,000
  • Net Yearly Cost Reduced from £1,164,542 to £540,000 (catheter kit + TUC Safety Valve)
Complication costs relate directly to immediate care required to manage urethral injury complications ie. additional bed days and ICU days incurred.

Using a reference of 142,000 in the NHS, assuming the above 0.55% incidence rate:
  • 1,704,000 catheterisations per annum @ £10 per catheter kit = £17,040,000
  • 9,372 urethral traumas per annum (1,704,000 catheterisations * 0.55% incidence rate)
  • Average 35 medico-legal claims @ £41,599 = £1,455,965
  • £75,336,742 in complication costs (83,627 additional bed days, 4,325.5 ICU bed days)
  • + £17,040,000 total catheter spend

Benefit to WM population:
As the manufacturing organisation (Class Medical Ltd) aspires to grow and develop an organisational structure in the UK to further promote and enable adoption of the TUC Safety Valve in the UK and Internationally, there is potential for an office base to be located in the UK with recruitment of a sales/clinical engagement team, marketing persons and a small administration and logistics team. This could benefit the WM region should the company decide to locate their office base in the UK as WM is a central location with access to a good logistical network and access to a skilled workforce.

From a patient perspective, through delivering upon the NHS ethos ‘Do No Harm’ by preventing: unnecessary adverse events, short and long-terms complications, unplanned care episodes, avoidable trauma, inpatient admissions, referrals to A&E and morbidity, the health and safety of the overall WM population (whom require TUC as part of their care) enables a better health economy whilst also reducing the strain on services in the region allowing a more efficient service to be provided. This in turn aids the local economy and resources can be allocated more efficiently and effectively to better support the needs of the population.
 
Current and planned activity: 
The TUC Safety Valve is pending CE and FDA Approval, both of which are on schedule for December 2019 as the final benchmark testing of sterility (shelf life) is finalised.

Commercial launch will take place internationally in January 2020. In the meantime, MedTech Connect Ltd are acting on behalf of Class Medical Ltd in the UK and internationally in engaging with KOL's, regulatory bodies, healthcare organisations and health innovation partners to identify pathway to adoption.

In the UK, we are identifying with AHSN partners regional sites for clinical evaluation and engaging discussions with community healthcare providers highlighting the safety and cost burden Urethral Trauma secondary to TUC in order for the TUC Safety Valve to be considered as part of the catheterisation pathway for patients and an effective value proposition to be delivered. NICE have also selected the TUC Safety Valve for selection by the selection committee to move forward with relevant guidance.
 
What is the intellectual property status of your innovation?:
Class Medical has a strong IP position, and no other syringe or catheter exists in the EU for TUC that has any safety features. The IP currently resides with the University of Limerick. The TUC Safety Valve was granted patents in the USA in October 2016 and the EU in January 2018. The simplicity of the design makes it very difficult to replicate. Importantly, the IP covers the device interface parameters with the human body and rate of inflation preventing a false positive signal. Parameters patented make it extremely difficult for other syringe or catheter designs to operate in the design window.

Innovation 'Elevator Pitch':
Tide is a social movement led by carers of people with dementia to have a voice in society.  Tide identifies and develops carers of people with dementia to support long term co-production and involvement with a range of health care providers.
Overview of Innovation:
Together In Dementia Everyday is a social movement led by carers of people with dementia. Tide enables carers and former carers to have a powerful collective voice, using their lived experience to raise awareness of their unique needs, have their rights respected and be empowered to influence policy, practice and research at all levels. Tide works with partners to ensure carers have a say and supports co-production at local, regional and national levels.

The Tide development programe creates trained and confident carers for health and social care to draw on as experts by experience to participate in local/regional planning forums and service reviews and to contribute to joint training. Carers can advise on and participate in research in health and social care, ensuring that the systems measure what really matters.

We have recruited and developed large co-horts of carers in the North of England who support a range of co-production activities with our healthcare partners.  Recently carers were invited to have a say in Westminister on the All Party Parlimentary Group on Dementia.

Getting involved also enables carers to influence change and shape their local communities by influencing how services are designed and delivered. Having a voice, understanding the system and being able to influence it improves carers’ well-being, enabling them to be strong advocates and care for longer

A person-centred approach must include involving carers as equal partners from the beginning; they know the person with dementia better than anyone else. Using their lived experience to inform policy and practice helps to improve services, ensures better outcomes for people with dementia, improves continuity of care, reduces carer breakdown and helps staff support people better. Carers represent 42% of the dementia care workforce so are key resources in achieving the shift from hospital to community.  Tide works with partners to give carers a meaningful say on service provision.

We would like to extend Tide into the Midlands - recruiting and developing carers of people with dementia and supporting them to have meaningful input into healthcare decisions at every level.

Benefit to NHS:
Carers of people with dementia have a unique experience of caring. This includes coping with the unpredictable nature of the condition and the emotional and psychological impact of grieving for the person they have known as a result of the cognitive decline of the person they are caring for;
 
Carers want to have their own voice and not be seen solely as adjuncts to those they are caring for. They should be recognised as experts by experience and helped to gain the knowledge, skills and confidence to influence and advocate for positive system change
 
There are 700,000 carers of people with dementia across the UK, with many feeling socially isolated and lonely as a result of their caring roles;
They make up the largest workforce (44%) in dementia, saving the care economy £11.6bn a year, but this is not recognised, valued nor respected by policy makers, professionals nor society as a whole;
 
Our Carer Development Programme will create a cohort of trained and confident carers for health and social care to draw on as experts by experience to participate in local/regional planning forums and service reviews and to contribute to joint training. Carers can advise on and participate in research in health and social care, ensuring that systems measure what really matters.
 
Giving carers a voice, improving their knowledge, skills and confidence and creating opportunities to get involved will help them lead healthy and independent lives. Research has clearly shown the adverse impact of caring on the health of those looking after people with dementia. Getting involved also enables carers to influence change and shape their local communities by influencing how services are designed and delivered. Having a voice, understanding the system and being able to influence it improve carers’ well-being, enabling them to be strong advocates and care for longer
 
A person-centred approach must include involving carers as equal partners from the beginning; they know the person with dementia better than anyone else. Using their lived experience to inform policy and practice helps to improve services, ensures better outcomes for people with dementia, improves continuity of care, reduces carer breakdown and helps staff support people better.

The UK has a growing and ageing population, with increasing numbers of dementia diagnoses, there are 850,000 people estimated to be living with dementia in the UK, this number is set to rise to one million in 2025. Additionally, one in five people will experience depression sometime in their life and the prevalence of delirium in people on medical wards in hospital is about 20% to 30% and can be much higher in intensive care units.

If you live with dementia, depression or delirium it can sometimes be really difficult to express what is important to you and why. The NHS recognises that it is incredibly important that people living with the 3Ds receive person centred, and individualised care, particularly as these can offer incorporated protected characteristics. 
 
 
Carers don’t apply for the job. In learning from scratch, they develop a wealth of knowledge and expertise about the person they care for. Not all carers are comfortable in the role. Tide helps carers articulate this and helps professionals providing care and support to understand this too.

Our Carers Development Programme increases carers’ confidence, knowledge and skills, validates their expertise and enables them to speak up to influence change. Modules include ‘Maze Runner’ – helping carers to find their way through the health and social care system, policy and legislation and to understand their rights. Tide helps carers channel their anger and frustration into constructive action so that they feel that they are making a contribution and are valued by society.

Working in partnership with people and building on their strengths and abilities require more than conversations with professionals through individual consultations. We need to harness carers’ collective voices to talk to and influence the system, policies and processes.

Tide connects carers together in a network, helping reduce isolation, giving them assurance that they are not alone and providing opportunities to get involved at whatever level suits them; this can be through an e-mail consultation, a letter to their MP, speaking at events, research or local, regional and national planning/advisory groups.

We currently operate in Northern England, Wales, Scotland and Northern Ireland and have over 600 members.

We propose working to recruit 50 carers of people with dementia in the West Midlands, providing them with 10 training and development modules and supporting them to have 100 opportuntities to influence.  This ranges from involvement on stakeholder groups, supporting CCG recruitment, co-producing projects, speaking at events, co-developing training and supporting reasearch.

We will then work with carers and partners to evaluate their input and to build long term connections so the voice of carers can be a resource that supports co-production in the long term.  Through the process we look to build stronger links between carers and the organisations involved to support robust co-production.

We know carers want to get involved with co-production and we would like to work with organisations in the West Midlands to give carers a worthwhile say on service provision.

Innovation 'Elevator Pitch':
Many physical examinations require patients to undress. The Bare-Non Modesty Apron protects patients modesty. This single use, cost effective, dignity restoring garment reduces feelings of vulnerability & embarrassment for clinicians & patients.
Overview of Innovation:
NHS policies on Dignity & Respect state that patients have the absolute right to be treated with dignity & respect in a way that suits their needs & that patient modesty must be protected at all times by the provision of suitable garments & drapes.
 
The use of a Bare-Non® Modesty Apron in an examination allows the patient to feel more comfortable & empowered rather than experiencing what is usually considered an uncomfortable & awkward experience. This helps to create a more harmonious environment between the patient & the practitioner.
 
Single use disposable, hygienic & hypo-allergenic, the Bare-Non Modesty Apron has a front split & extensible neckline, meaning one size fits all. It is simply placed over the patient’s head prior to examination or treatment & covers the front torso to mid-thigh level.


 
Each Apron comes individually packed with a ‘Proof of Use’ sticker which can be signed & inserted into the patients notes or attached to other documentation such as an ECG tracing. Lightweight, comfortable & moisture resistant it allows quick unhindered access to the patients’ chest & abdomen should the need arise.
 
Over time, clinicians can become desensitised to patient’s in a state of undress, a condition Bare-Non identify as “Nudity Blindness”. This can leave patient’s feeling vulnerable & embarrassed & unable to participate fully in important aspects of their consultation such as shared decision making whilst they focus on their state of undress. Patients who have worn the Bare-Non Modesty Apron have reported feeling less inhibited, more empowered & more able to participate in their treatment or consultation without feeling the need to rush to put their clothes back on.
 
Current garment offerings have to be completely removed in a variety of settings, leaving the patient inadequately protected in terms of modesty & dignity. Sometimes clinicians are only provided with drapes, which have a tendency to fall off. Cases of compromised patient modesty can lead to patient distress & formal complaints, the impact of which can be personal, organisational & financial with devastating & long-lasting consequences for both patient & clinician.

The benefits of employing the Bare-Non Modesty Apron within the NHS are:
  1. Positively affects patient satisfaction and outcome measures, and shows a real commitment to understanding and acting upon Dignity and Respect concerns – compliance with relevant NICE and NHSE guidance etc.
  2. Supply chain efficiency improvements in terms of reducing laundered garment use.
  3. Financial in terms of cost savings over current garments and reduced claims for breach of dignity and respect.
In spite of the pressures the NHS comes under, Dignity and Respect in patient care is acknowledged as a critical component in caring for patients. Finding innovative, cost effective ways to further enhance the patient experience may have a favourable effect on outcomes related to Regulation 10. Patient satisfaction is an important aspect to the NHS as evidenced by the Patient Survey Programme which collates data into Overall Patient Experience Scores

The Bare-Non Modesty Apron, if measured against a standard textile or single use hospital examination gown is more cost effective. However, from research carried out by Bare-Non it is evident that standard textile examination gowns (36p per use) and single use disposable gowns (Medline Examination Gown 51p per use) are unsuitable garments for many of the examinations and procedures they are used for as they require removing in order for the clinician to perform examination or treatment. The drapes employed in some NHS departments are unfit for purpose as they do not stay in place and can lead to more claims of compromised patient modesty.

The Bare-Non Modesty Apron is presented as an individually wrapped item in a paper pouch. Its small footprint and handy box size means storage is easy in a variety of clinical settings. Being single use disposable has supply chain advantages over laundered products, as there is less movement and storage requirements are smaller.

Innovation 'Elevator Pitch':
We have successfully designed, implemented and evaluated a standardised triage system for use within maternity care.  It is currently used in eight maternity units, with more planned.
Overview of Innovation:
Triage systems are designed to ensure the patient receives the level and quality of care appropriate to their clinical needs and resources available. Triage involves establishing the presenting problem, undertaking a standardised physiological assessment including vital signs and results in a score being assigned based on predictors of urgency which determines the priority of on-going care.
The physiological changes associated with pregnancy mean the standard triage tools may not be applicable. In addition, the underlying good health of the maternity population, which may mask the severity of maternal illness, and no assessment of the condition of the unborn baby reinforce the need for a specific maternity tool. While some informal triaging takes place within maternity care, a national survey showed that women currently wait to be seen in the order in which they arrive and that current prioritisation systems do not use clinical determinants.
 
Birmingham Symptom specific Obstetric Triage System (BSOTS)
The system was co-produced by clinicians and researchers in Birmingham to prioritise care for women presenting with pregnancy related complications or concerns.  
The BSOTS bundle includes:
  • Completion of a standard clinical triage assessment by a midwife within 15 minutes of the woman’s attendance.  This includes taking a brief maternal history, completion of baseline maternal observations, assessment of pain levels, abdominal palpation and auscultation of the fetal heart rate .
  • This assessment is used to define a category of clinical urgency using a 4-category scale, which guides timing of subsequent assessment and immediate care (by an obstetrician if required) using algorithms.
 
  • Standardised symptom-specific algorithms are used for allocation of clinical priority and the immediate care and further investigations of the eight commonest reasons for attendance (abdominal pain, antenatal bleeding, hypertension, suspected labour, ruptured membranes, reduced foetal movements, unwell/other, and postnatal concerns). 
 
  • Documentation is provided to support and standardise completion of the clinical tasks required.
 
Implementation of BSOTS requires multidisciplinary training of clinicians which takes approximately three hours. The BSOTS training has also been developed and elevated by the team

The benefits to the NHS of a dedicated maternity triage system are centred on improving safety for women and their babies by reducing the interval from their attendance to their initial review by a midwife and prioritising urgent care. BSOTS is supported by Royal College of Obstetrics and Gynaecology and Royal College of Midwives for national implementation.
Implementation of BSOTS as the system improves management of the triage department by enabling staff to:
  • See how many women are waiting who have not yet had their initial triage assessment to determine level of clinical urgency
  • See how many women who have had the initial assessment and the level of clinical urgency is known for each women
  • Know when further assessments are due for women in the Department
  • In circumstances where women attend who require urgent treatment it allows women with less clinical urgency to be safely moved out to the waiting area and escalation to occur
  • Facilitate easy handover of the department between shifts using SBAR
  • Use a shared language between clinicians to communicate clinical priority
BSOTS  provides a standardised process to manage emergency attendances and reduces staff safety concerns in maternity triage whilst increasing their resilience and improving handover and communication.
 
Initial evaluation at BWCNFT demonstrated  that the use of BSOTS increased the numbers of women seen within 15minutes of attendance from 39% to 54% [Relative risk (RR) 1.4 (1.2, 1.7 (95% confidence interval (CI)] and reduced the time to medical review for those who require it. Thus providing a robust pathway for rapid assessment and urgent medical care if required, improving outcomes for mothers and their babies.
Good reliability and validity of the BSOTS bundle were found on the basis of the numbers of women who re-attended and investigation of predefined maternal and neonatal morbidities.  All the midwives reported that BSOTS training had improved their knowledge and confidence and that implementation of the system improved safety for mothers and babies and reduced stress amongst the clinicians.
Intra-rator reliability was excellent both at BWCNFT initially with an Intracluster Correlation Coefficient (ICC) of 0.961 (95% CI 0.91-0.99), and in the additional maternity units where the ICC was 0.971 (0.915-0.996). There was no apparent difference between midwifery band level and amount of triage experience. Showing that the BSOTS tools are reliable and consistent for use amongst varied midwifery staff.

The West Midlands remains a national outlier for poor rates of perinatal and infant mortality and is committed to the national ambition to halve the number of stillbirths, neonatal and maternal deaths and brain injuries by 2025.
 
In line with the National Maternity Review (published in February 2016) which set out a clear vision for maternity services across England to become safer and to promote good practice for safer care; the Birmingham Symptom specific Obstetric Triage System (BSOTs) improves the process and clinical prioritisation of care for women presenting with pregnancy related complications or concerns. BSOTs has also been shown to prioritise urgent care for those women who are most unwell or requiring more rapid access to obstetric intervention and emergency treatment.
 
With increasing use of the Birmingham Symptom specific Obstetric Triage System (BSOTs) across the maternity units within the West Midlands region, women can expect continuity of process and care when they present to units with urgent concerns and that care is standardised across the region. Standardisation of the maternity triage pathway also provides an opportunity to ensure that the midwifery and medical workforce in maternity are able to be trained with the right skills to implement appropriately timed and clinically relevant prioritised of care within all maternity triage departments in which they may work.
Current and planned activity: 
The Birmingham Symptom specific Obstetric Triage System (BSOTs) is currently used in seven units nationally; including half of all maternity units within West Midlands.
A further twelve maternity units have received training for their implementation teams and are due to launch the BSOTs programme in the next six months.
Bespoke training courses for maternity teams are held in Birmingham every two months. This training includes specific training the trainers materials, the BSOTS© maternity algorithms, Triage Assessment Cards and Patient information posters and tips for implementation from other units.

Further development of the Birmingham Symptom specific Obstetric Triage System (BSOTs) with Clevemed systems to transfer the paper based system into an electronic tool with contemporaneous patient record is on-going; due to complete by May 2019. This will enable maternity units currently using the Badgernet Maternity electronic system to use BSOTs digitally and further improve availability.
What is the intellectual property status of your innovation?:
Use is currently protected by signing a Licensing agreement provided by UoB and MidTech.
 
The schedule within the licensing agreement includes the Birmingham Symptom specific Obstetric Triage System (BSOTs) package for Training and Implementation and is intended to ensure the package is used as intended and that training has been given before implementation
The package is intended to support local training, implementation and on-going use of the (BSOTs) system, in the maternity unit assessing women who attend with unscheduled pregnancy related concerns. This package is set out during the face to face training course, it includes specific training the trainers materials, local training materials, the BSOTS© maternity algorithms, BSOTS© Triage Assessment Cards and BSOTS© Patient information posters. The components of the triage system should be adopted in its entirety, rather than separately.

Innovation 'Elevator Pitch':
The vPad allows clinicians to remotely monitor and manage the long-term health of residents in care homes by establishing long term health baselines.
Overview of Innovation:
The vPad is tablet sized device that features a number of attachment modules that allows the transfer of proxy biophysical data into a GP IT clinical system.
 
The system supports the establishment of long term health baselines for care home residents - enabling earlier detection of deteriorations in health and improving decision making from care home staff regarding when to seek emergency health care - potentially avoiding unnecessary hospital admissions and emergency GP visits by integrating the NEWS2 scoring system.
 
We believe the solution ties in well with NHS England's Enhanced Health in Care Homes framework. Our system bridges the communication and medical data transfer gap between primary care practices and care homes. We are integrated with EMIS and have TPP interoperability in the development pipe works too.
 
The system has two components:
  1. Equipment. vPad, an All-in-One vital sign smart monitor. The device measures Blood Pressure, SpO2, Pulse rate and Temperature. It also has a built-in 12-lead ECG for atrial fibrillation detection.
  2. Software. 121 Sync Agent connects the GP clinical system with vPad through Wi-Fi/3G and synchronises vital sign measurements to the Electronic Patient Record. It displays an on-screen historical vital signs baseline.
Currently Knowsley CCG are piloting the system with care homes and we're in talks with a number of others.

With technology, medicine, healthcare and lifestyles ever improving, people are living longer and longer than they used to.

A projection made by the office for national statistics (ONS) states that ‘By 2041, the 1960s baby boomers will have progressed into their 70s and 80s, and by 2066 there could be an additional 8.6 million people aged 65 years and over in the UK’. At this stage the UK’s 65+ age group would account for 26.5% of the population.

The NHS estimates that roughly 1 in 7 OAPs (65+) currently reside in a care home. Based on the aforementioned projection this number will likely rise in the near future.
The issue will be more pronounced in the West Midlands area with the ONS currently estimating that OAPs make up 18.3% of the total population (2016 population estimate).

At age 65 years, both men and women can expect to spend around half of their remaining life expectancy in good health. However, the likelihood of being disabled and/or experiencing multiple chronic and complex health conditions among those aged 65 years and over increases with age. As life expectancy increases, so does the amount of time spent in poor health.

Healthcare requirements increase with age, with healthcare costs increasing steeply from around age 65 years. Hospital admissions have increased since roughly the beginning of 2007, but with a steeper increase in admissions for the 65+ age group. This has contributed to rising healthcare costs.

A study carried out by Stoke and the North Staffordshire CCGs featured in the Care Home Strategy (2015/17) found that on average 230 (Northern Staffordshire) care home residents were admitted to hospital each month. In 2013/14 there were a total of 2804 admissions which costed the NHS £6.73 million with an average length of stay of 6.94 days.
With relevance to reducing unplanned admissions and A&E attendances local data analysis demonstrated that approximately 30% of unplanned admissions from care homes could’ve be avoided with improved care planning and proactive management or timely access to specialist services.

By implementing our solution we have the potential to better manage a growing ageing population who by default will have increasing dependency on an ever strained healthcare service. Through the prevention of illness, where possible, therein lies the potential of saving vast amounts of both time and money.

We are currently focusing on approaching CCGs, as care home services are often commissioned through CCGs it also makes sense to approach these organisations so that they can see first see the cost effectiveness of our solution.

We have also networked through various conferences, which has allowed us to reach out to a number of clinicians who have shown interest in the vPad.

We are currently trialling with Knowsley CCG and are due to start with another practice imminently. 

We’re looking to trial with other organisation and clinicians so that they may also see the benefits of both our solution and investing in technology to enhance the provision of healthcare.

Teleswallowing enables poorly people to be seen more rapidly by specialists and improves staff productivity threefold
Overview of Innovation:
Teleswallowing is an approach using technology whereby a speech and language therapist can run a virtual clinic to triage, manage and review his/her patients with eating and drinking problems (dysphagia).  Teleswallowing Ltd staff train the carers within residential and care settings to be able to link remotely with speech therapists to provide rapid access to specialist skills to avoid unnecessary distress, avoidable deterioration and hospital admissions.  Speech and Language Therapy is a scarce resource and the waiting lists to see a specialist are long.  The use of Teleswallowing technology prevents vulnerable people suffering and can be used as an out of hours/weekend cover approach.

I developed the approach at Blackpool where it is used; NWAHSN sponsored its development in Cheshire and West Hants CCG has commissioned the use in Southampton.  We have proven, through clinical trials supervised by Dr Liz Boaden from the University of Central Lancashire, that the approach increases staff productivity three fold and saves approximately £60 per assessment.  University College London used the approach in a workforce scoping project and found that 10.38 weeks of speech therapy time could be saved by using Teleswallowing.  Please see all evaluations on www.teleswallowing.com

Benefit to WM population:
Many SLTs (speech and language therapists) are women working part time and below their level of training due to having child care or elderly parent responsibilities.  We would like to upscale our approach so that we not only offer training in the approach, but are able to deliver commissions for weekend/out of hours cover remotely, giving the opportunity for West Midland SLTs to work from home at hours best suited to their life and also help reduce the waiting times for sick people needing dysphagia assessments.  The SLTs would earn a better income, use their hard earned skills and be of benefit to the health system - by utilising a modern, digital approach.
Current and planned activity: 
The approach is used at Blackpool Teaching Hospitals, Cheshire and West Hampshire CCG.  I am bidding for a Women in Innovation award (Innovate UK) in order to develop the approach into an e-learning package, an app which then will be a digital clinic portal whereby SLTs will book in their availability to fulfil contracts remotely.  The SLTs will also be able to take advantage of the e-learning videos/second opinions/webinars which I intend to develop if I win the funding which goes with the Women in Innovation bid.

Watson for Genomics provides peer-reviewed, molecular evidence to help oncologists create personalized treatment strategies for patients based on results from genetic testing of tumour tissue.
Overview of Innovation:
Watson for Genomics analyses and categorizes genetic alterations that are related to cancer progression and provides a list of potential therapeutic options that target each alteration. The interpretation is done within minutes, and a comprehensive report is generated. The summary page of the report lists all the critical genetic alterations that were identified in the analysis and associates each with therapeutic options. Levels of evidence, extracted from relevant literature are provided to show the strength of the evidence.
Watson for Genomics presents findings with evidence via hyperlinks to sources for easy drill down. The drug report section provides details of identified drugs with overview, rationale for the association and level of evidence, as well as recruiting clinical trials.
The molecular profile interpretation provides in depth detail of the known genetic alterations found in the tumour sample. For each mutation, molecular and scientific evidence explaining the pathologic relevance of the alteration is provided. The diagram offers easy viewing of mutations with chromosomal location.

There is a legislative requirement to store critical medicines within specified parameters. Moreover, it is imperative that safe storage can be demonstrated and that only safe products are used.
 
Equally, any fault or disruption to the storage infrastructure doesn’t automatically render the products beyond safe use. Smart monitoring, reporting and validation afford peace of mind. Efficacy of the drugs can be evidenced and detect abnormal patterns of behaviour or access issues e.g. refrigerator door left open.
 
Broad clinical guidelines indicate that regular, period monitoring by technicians will suffice; often once every 24 hours and often not during weekends and holiday periods.
 
There are several risks with this approach that can render expensive drugs ineffective and worse still, fail to deliver the anticipated results. A power outage in the early hours or over a weekend may be masked by a reinstatement long before working hours. The damage may already be done if excessive temperatures have been realised.
 
The solution is to introduce smart monitoring with remote data logging and alerts. Continuous, electronic monitoring not only captures specific events 24/7 and raise alarms but provides valuable trend data. Battery backup will buffer mains failure without disruption to the monitoring or connectivity. Real time monitoring has clearly shown systematic issues occur out of working hours and variations within specific refrigerated facilities. Clinical practices will also affect the storage conditions; a feature that is evident from trend data. The effects of prolonged loading and unloading with the refrigerator open shows the impact upon refrigeration temperatures and the extended recovery time to achieve safe storage temperatures when eventually closed. Access to the refrigerators out of prescribed hours may also raise an alert if there is a security concern.
 
EveryWare has installed local monitoring systems in hospitals and clinical environments that are capable of tracking a range of critical conditions. Not just in refrigeration units but ambient conditions in key locations may be monitored through a common network. Within treatment, theatre, storage or living areas the network of wireless devices can track temperature, relative humidity and pressure along with other factors such as controlled access and refrigeration performance.
 
In homes EveryWare could be used with ambient and movement sensors to detect risk of hypothermia or falls.

Positive actions were taken to improve the effectiveness and avoid potential problems that might have occurred. Clinicians benefit from the reduced workload and the availability of high resolution data proves compliance with healthcare guidelines 24/7.
 
We have recently entered discussions with GP practice in Norfolk in response to CQC’s strict environmental monitoring requirement for drug storage.

Innovation 'Elevator Pitch':
Flo-Tone CR is a tool helping patients acquire good Maintenance & Technique in using Pressurised Inhalers, teaching how to Inhale SLOWLY and STEADILY. Inhaler Technique Skills are important and a major theme in BTS, GINA and NICE guidance.
Overview of Innovation:
Flo-Tone MDI (also known as Flo-Tone CR) uses a ‘Positive’ Coaching Whistle to help patients learn to inhale SLOWLY and STEADILY. Whistle Signal is a prompt for the canister to be pressed releasing the medication & the duration of the Whistle helps the professional to coach the patient towards correct use.
Pressurised inhaler mouthpieces come in a variety of shapes, accordingly Flo-Tone was originally produced in ‘Circular’ & ‘Regular’ Models. A new improved Flo-Tone CR now fits all mouthpiece shapes by fitting inside the pMDI mouthpiece. The new design ensures all pMDIs produce a whistle at the same flow rate, helping standardise technique.
Studies have been conducted with the new Flo-Tone to ensure that it delivers the full dose. The first study detailing the development of the improvement & its performance was presented at BTS 2015 & subsequently at DDL26.
Available on prescription, the improved Flo-Tone CR can be used with a pMDI simulator (Trainhaler) for training & with the Patient pMDI. It incorporates a cap so that it can be left in situ on the Inhaler. It also includes a rim on the mouthpiece to help patients get their teeth out of the way of the aerosol spray.
We have tested Flo-Tone CR with a variety of inhaled medications (Flutiform, Clenil, Ventolin, QVAR and Sirdupla) – in each case the respirable dose (ie that part that reaches the lungs) delivered for the medication plus Flo-Tone closely matched that of the device alone & in each case unwanted throat deposition was reduced.

There have been many papers published, detailing the misuse of pMDI among patients;
Clement Clarke International have used their expertise to concentrate on this area of products. This
has led to the introduction of the ‘Inhaler Technique Training’ range. Each device is targeted at the
training of inhalation technique, guiding patients to inhale at the correct flow rate for pMDI use.
 
Inhaler technique errors occur in the hands of patients and healthcare professionals. It has been
demonstrated that the majority of healthcare professionals cannot demonstrate correct inhaler use
to their patients. It is not therefore surprising that patients are mostly unable to demonstrate good
inhaler technique. The consequences are significant; patients take higher doses to compensate for
lack of efficacy from medication lost through poor technique, this results in poorer control,
hospitalisation and increased healthcare costs.

The West Midlands region provides a great opportunity for rapid realisation of the benefits of improved inhaler training from a clinical, economic and patients’ quality of life perspectives.

All the required products and training materials are currently available for instant deployment and would offer significant benefits to those GP practices or secondary care providers willing to challenge existing ineffective training and delivery mechanisms and who would act as either the Regional or National adoption lead.

Planned/required activity
As a Manufacturer, Clement Clarke undertakes an extensive R&D program and also undertakes On-Site Clinical Trials with GP Practice & Secondary Care.
However, research and trials support partners would be welcomed to further develop the evidence base demonstrating the benefits of improved staff training with associated patient benefits.
In addition, Health Economic studies would also be welcomed to quantify the medication and admission cost savings that would be achieved. This would supplement the Isle of Wight study and include a wider geographical and ethnic / demographic patient cohort.
