Innovation 'Elevator Pitch':
MediShout uses Apps and AI to turn hospitals in smart, automated buildings. Our technology prevents infra-structure and logistical problems (e.g. broken IT, missing stock, faulty equipment) from delaying staff who can focus more time on patient care.
Overview of Innovation:
THE PROBLEM
IT, Equipment, Facilities, Estates, Stock... clinicians rely on these logistical Departments to deliver care, but often don’t report problems with them because:
1) Reporting-channels are too slow e.g. it often takes over 10 minutes to call a Helpdesk, and some Helpdesks get over 100,000 calls annually!
2) Staff don’t know how report issues as each Helpdesk has a different contact number, bleep, email etc
3) Sometimes NO reporting channel even exists

THE SOLUTION
MediShout solves this huge problem by giving staff a single-interface App to instantly report ANY logistical issue they encounter, such as broken IT or faulty equipment. Users simply select their ward, type their issue and press Shout. Our algorithms send this information to the correct administrator or Helpdesk who create change. Users can attach photos which helps Engineers fix problems faster. Two-way messaging and feedback provide regular updates to users on their issue.

MediShout captures data on the CLINICAL impact of each logistical problem reported; allowing hospitals to triage and fix the issues impacting patient care first. Our artificial-intelligence (machine-learning) algorithms can predict in advance when problems will occur, such as broken equipment, so hospitals can perform planned maintenance and prevent down-time in service provision.

BENEFITS
MediShout brings an excellent Return-on-Investment. Independent analysis by Health Enterprise East and Eastern AHSN, shows our platform can save NHS Trusts over £1million in efficiency savings per year. It can also generate direct cash-releasing savings, for example by requiring fewer administrative staff to operate Helpdesk telephones.

We improve the working environment and morale for staff, and can save 15 minutes per clinician daily.

Patient care improves as staff have more time with them. Our case studies also demonstrate faster patient discharges occurred from improved work-flow.

INNOVATIVE
MediShout is globally the first product to integrate all logistical Departments onto one platform, giving staff a quick single-interface App to report issues. We are the first to collect real-time data on the clinical impact of logistical issues whilst using AI to predict problems.

Developed by Registered Nurse, Multi Award Winning Neo-slip® is a novel, innovative yet practical solution that aids application of compression stockings (TEDS)

Low friction Neo-slip® design creates a smooth foundation with a lubricious effect, on which to slide TEDS in place. Available in color-coded sizes, Neo-slip® enhances the comfort of application which enhances the comfort for patients. 

Neo-slip is easy to implement in a ward situation with improvements in patient experience. The use of Neo-slip improves patient safety, as the use of Neo-slip replaces the need for nurses to use plastic bags to ease application of compression garments (as many nurses do). 

Having won 21 awards and an appearance on BBC’s Dragons Den, this is a good quality British made innovation.

Benefit to NHS:
Using a device to help with the application of anti-embolism stockings, often called thromboembolic deterrent stockings (TEDS), can potentially facilitate greater adherence to the use of stockings, and potentially reduce the risk of deep-vein thrombosis (DVT). 


Guidance from the National Institute for Health and Care Excellence (NICE) (2010) recommends that, when patients are discharged with anti-embolism stockings, they understand the benefits of wearing them and the need for daily hygiene removal.

In a 2015 study in a London trust was undertaken, where 35 nurses were interviewed. 
Most of these 35 staff were involved in changing, measuring and applying stockings on a 
regular basis. Fourteen spoke of the time it took to put stockings on and 32 spoke of difficulty in application: ‘it is a massive struggle’ and ‘it requires all my strength’. One of the main issues appeared to be pulling the stockings up over the patient’s heel. 


On average without the Neo-slip device the application of stockings took between 65 and 120 seconds. 
Neo-slip enhances patient experiences and increases patient safety (potential skin damage and infection control issues if a plastic bag is used).

Anyone receiving care and support within an NHS hospitals will value independence highly, as it brings with it dignity, control, self-esteem, and fulfillment. When caring for an elderly person, whether in a hospital or in their home, independence is key to ensuring happiness and improving the quality of life.
 
It is therefore imperative that in a care setting such as a hospital or visiting district nurses (in the patients home) will encourage independence as much as possible, in all aspects of life and daily activity.
 
This means enabling and supporting the person in your care to maintain an active mind and body as much as possible, within their abilities, whether that is something as simple as applying their own stockings or making a cup of tea.
Current and planned activity: 
We will attract the attention of those who need to wear anti embolism or compression stockings and the hospital nurses or carers and relatives.

We will embark on a Marketing Campaign that is invested in the current Social Media Marketing Environment. The campaign will be designed to increase awareness of the way in which utilising our product will improve the quality of life.

Our new website is due to launch in January 2018 and we plan to create a variety of information leaflets.  We are developing posters to provide to the hospitals and we have a sales team who will facilitate the introduction of Neo-slip into new ward areas.

Measures:
The success of our innovation can be measured by how quickly the Neo-slip usage increases on wards within a hospital. 
Patients’ views on their care are an essential component in assessing the quality of our product. We measure patient outcomes using patient-reported outcome measures (PROMs).  This provides a clinically relevant and scientifically rigorous resource which also includes the patient perspective in the subsequent evaluation.”
According to Nice (2017), ‘patient education will increase adherence to prescribed DVT treatment and help reduce further costs from hospital readmissions. 
We measure the increase in usage using our own IT equipment starting with quantity numbers and accounting reports from ‘Sage 50’.
Our reporting and invoicing software also measures cash flow and vat in relation to the growth of the business per sale.
Schedules for time and resource are managed by our business administrator who manages the project to add wards to our client list taking into account our ability to guarantee delivery estimating any in built uncertainty. 
The NHS guidance on flow charts to document all our processes in an efficient manner is used throughout the creation of our processes.
As the stakeholder requirements are almost identical being NHS wards, and the product itself is fully tested this management is not taxing.  If we require further processes to be added we will use a project manager.
The success of our innovation can also be measured from patient feedback and the uptake of patient champions spreading the word to other patients and healthcare professionals.

Overview summary:
The Multi Award Winning Neo-slip® is a novel, innovative yet practical solution that aids application of TED (thromboembolism-deterrent) stockings in a ward environment or whilst the postoperative patient is recovering at home.
Challenge identified and actions taken :
After seeing both nurses and patients struggling to apply compression garments in my role as a registered nurse I experimented with different materials in the hope of creating a smoothing aid that reduced friction and helped in the application of tight compression.

The result? The Neo-Slip.

Neo-slip® is a new, innovative low friction pouch available in colour coded sizes and fits onto the limb before the tight fitting compression, this creates a lubricous/slippery effect which slides the stocking into place and makes putting on stockings as easy as applying socks.

Neo-slip® enables patients to live independently and improve their quality of life thus enabling staff to focus on other tasks. Patients in the community no longer have to rely on nurses or social workers coming into their homes to apply tight surgical stockings.

Impacts / outcomes: 
Neo-slip® delivers value for money by reducing the significant costs associated with DVT for the NHS, the economy and individual patients, which could also affect their family.
 
Health Select Committee estimated in 2005 that the total cost including the indirect cost to the UK for the management of DVT was approximately £640 million. In addition, the total annual costs of treating venous leg ulcers, a consequence of DVT, in the UK were in the region of £400 million.

Cost of treating complications is also very high; if you compare it to the cost of purchasing our product Neo-slip®, this will go on to encourage patients to wear their stockings that are vital in the battle against DVT.

Estimates show cost of treating a patient with DVT is £216 per day. With a typical stay in hospital of 5 days bringing total cost £1080 for just one occurrence of DVT.

Neo-slip® is sold to the NHS at £5.80 per unit and saves considerable time (and hence costs) of applying TED stockings, significantly reduces the cost of additional treatment and expensive stays in hospital (and thus increasing bed availability for serious illness) but most importantly helping nurses and patients, particularly when at home to apply their TED stockings correctly and easily.

Our strategy fits with current NHS strategy and objectives in driving efficiency and tackling waste, to make money invested in the NHS go further by delivering the services that patients want, including the latest technology.

Our innovation supports this by encouraging more efficient use of TED stockings. For example, a hospital purchased 27,779 pairs of anti embolism stockings at a cost of £68,158. However, Quinn et al (2015) surveyed 60 patients and found that over half found difficulty in the application of TED stockings, whilst physicians predicted that difficulty in the application was the main reason for non-adherence.

Anecdotal evidence suggests many patients do not adhere to the advice given in hospital to wear stockings for up to 6 weeks following discharge due to the difficult application.

Our innovation fits with current NHS strategy and objectives because the low unit cost pricing matrix of our innovation will support the NHS to take further action nationally to ensure that they can deliver more benefit for patients from every pound of its budget. As well as harnessing people power the NHS needs to leverage the potential of Innovation, which enables patients to take a more active role in their own health and care

Plans for the future:
We are expanding our range to include children size Neo-slip. This is a direct result of requests from nurses in the lymphedema clinics
Tips for adoption:
In the past two years, I have completed the design registration for Neo-slip®, trademark, patent pending, have obtained ISO9001 recognition, gained availability via NHS drug tariff (prescription) and, earlier this year, our nurse-led company achieved accreditation and listing on the NHS Supply Chain.

Listing on the NHS Supply Chain is vital for the future growth of the company, as this listing allows all NHS hospitals to order Neo-slip® without having to undertake their own due diligence on a product's efficacy and quality.

We currently supply  private hospitals across the UK and recently have signed an agreement with Boots chemist to fulfill our prescription orders for community patients.
