Innovation 'Elevator Pitch':
Spring Active has developed an integrated digital and physical back pain coaching approach for the management of back pain.  We will drive improved efficiency of care for providers whilst reducing costs of delivery.
Overview of Innovation:
Spring Active has developed an integrated digital and physical back pain coaching approach for the management of back pain. Our treatment combines evidence-based clinical approaches with innovative internet and mobile platforms. This early intervention approach will result in reduced waiting times for patients, and enable patients to be in control of their care, having immediate access to self-management leading to improved compliance and improved outcomes. In turn, we will drive improved efficiency of care for providers whilst reducing costs of delivery.

HealthyBack Coach™ is a digital and physical “virtual” service that combines digital screening, HealthyBack™ workbook and remote video consultations with a qualified physiotherapist (Spring Active Back Coach).
 
Here’s how it works:
  • Following referral, Spring Active contact the patient immediately to arrange delivery of materials and consultations – All materials are provided digitally and all consultations are via remote video link.
  • A member of our Clinical Team (Physiotherapist) carries out the remote video assessment & consultations each week for 3 weeks.
  • By week 3 the patient has a recovery plan that’s right for them and the tools to ensure a full recovery.
  • All referrals are registered on digital outcomes in order to record and monitor their improvement over 5 years
This service is designed to improve access at scale, reduce waiting times and reduce costs for healthcare providers, by providing instant access to the expert help & advice needed for an efficient recovery from an episode of back pain.

The Spring Active programmes involve exercise and educational sessions, with an emphasis on working towards purposeful individualised goals and developing skills to improve back pain in the long term. In order to support long lasting improvements and functional gains the patient will also go through digital outcomes measures for 5 years.
 
Our programmes focus on self-management improvements through identifying unhelpful pain beliefs and behaviours, providing tailored exercises to improve movement and body awareness, psychological support and practical coping and problem-solving strategies.

Our programmes have been developed in line with Spinal Taskforce and NICE guidelines.

Benefit to NHS:
Back pain is reported to be the largest cause of disability in the world. This has a huge impact, both on the economy with lost working days, and on the health systems trying to cope with demand leading to long waiting lists and high healthcare costs. Although the web is often the first port of call for people, with “back pain” the number one searched for complaint, web advice is patchy and often not evidence-based. It is estimated that those in chronic back pain consult their doctor up to five times more frequently than others according to the National Office of Statistics and that those with back pain have health care expenditures approximately 60% higher than individuals who do not suffer from back pain.

Our treatment combines evidence-based clinical approaches with innovative internet and mobile platforms. This will result in reduced waiting times and enable you to be in control of your care. Having immediate access to self-management leads to improved outcomes.

Benefit to WM population:
As a West Midlands based SME, adoption of the HealthyBack Coach within the region would allow the company to grow and take on additional specialist staff. Our current forecast suggests the creation of 45 jobs over the next 5 years in software engineering, e-commerce agencies, HealthyBack physiotherapists and sales and marketing.

Reduction in travel for patients (reduces pressure on the region’s transport infrastructure) a reduction in pressure to the primary Care system.
Current and planned activity: 
Due to lack of progress within the NHS Spring Active is currently trialling and developing the HealthyBack Coach in 2 areas;
1.     Direct to consumer trial – this is providing the opportunity to gather user feedback and develop the approach to suit the patient
2.     Trials with large insurance companies to provide HealthyBack Coach direct to client company employees

Spring Active is keen to work with NHS to develop case studies in order to demonstrate financial benefits specific to the NHS and support the develop of care pathways for the management of back pain.

Overview of Innovation:
Silhouette is an easy-to-use wound imaging, 3D measurement & documentation system providing accurate wound information at the point of care to support effective clinical management of patients. Silhouette wound informatics are available as a wound assessment PDF report and on a secure web-enabled database and for easy access and sharing across the entire care team. 

The wound images and wound healing progress charts are highly visual and provide immediate and clear indication of a patient's response to treatment.

This award-winning system includes an imaging 3D camera that precisely and consistently measures the area, depth & volume of wounds to track and report healing progress. Silhouette enables wound assessment activity and wound outcomes to be monitored, providing wound information that is available instantly to key stakeholders, integrated with your EMR system. Silhouette is proven in both clinical research & clinical practice.

Silhouette supports pro-active management of wound-related risk & improves communication, helping practitioners to be more productive & effective in their delivery of wound care. Silhouette is suitable for all settings, including acute out-patient clinics, community-based care, home health & telehealth wound care.

The Silhouette system is made up of three main parts – a point of care wound camera, smart software for measuring wounds, & a wound information database:

Currently, the burden of wound care on the NHS is estimated at £3billion per annum, with unacceptable variances reported in practice standards and outcomes achieved so that thousands of patients are not receiving the standard of care that should be possible.

Silhouette technology transforms how wound status is assessed, documented and reported. Better quality of wound information available in a connected and interoperable system means that clinicians are better supported for effective and timely clinical decisions and actions. Silhouette facilitates data sharing across an entire care pathway to reduce the risks of fragmented care across silos.

Silhouette equips, empowers and enables healthcare professionals and patients to collaborate for best practice to achieve optimal wound outcomes.

As well as assuring patients of best practice and quality of care, patients can actively engage in their care plan and outcomes due to the highly visual nature of Silhouette wound healing data and charts displayed on the software screens.

The benefits of rapid and effective wound healing for the patient should not be underestimated. The psychological consequences of living with a chronic wound are well documented. Upton and Hender (2012) suggest that patients with chronic wounds may experience negative emotions such as:

  • Lack of self-worth

Entec Health is an appointed distributor to ARANZ Medical for the Silhouette solution in the UK. Our partnering goals are to inform & engage NHS stakeholders on the advantages of adopting digital wound assessment & wound informatics to address the “best practice, clinical excellence & cost-effectiveness gap” in wound management. Entec has supported 5 field Silhouette evaluations in acute & community care with considerable investment of time, expertise & resource. Entec has successfully established early adopters of Silhouette across the NHS:  
Clinical Practice: King’s College Hospital London Diabetic Foot Clinic, North Bristol Trust Enablement Centre
Clinical Research: 10 NHS centres of excellence in diabetes foot care are currently using Silhouette in a multi-centre clinical trial sponsored by EdixoMed

The Silhouette solution is supporting wound outcomes documentation, reporting and informatics for effective and efficient wound care service delivery. Silhouette is enabling transformation of wound care pathways in acute, community and integrated care settings. Early market for Silhouette is the US with UK, Europe and Asia as emerging markets.  
Investment activity:
Entec Health, as an appointed distributor of Silhouette in the UK,  has invested considerable time, effort and funds to introduce the Silhouette to the NHS/UK market since 2011. ARANZ Medical, the developer and manufacturer of Silhouette has invested signficant funds, effort and expertise in R&D and bringing the Silhouette solution to the global market.

An enterprise level Silhouette system based on 5 Silhouette assessment devices and SilhouetteCentral database package starts from £45,000 (net, excluding VAT) for a capital purchase plan. Data integration, IT services, project management and annual software maintenance/support fees are additional costs, dependent on requirements. Silhouette pricing can be offered as an annual subscription model or a capital purchase model to suit different budgetary requirements.

Innovation 'Elevator Pitch':
Virtual Clinic is an innovative mobile app which securely sends real time patient data to the hospital database.  A web interface allows the consultant to review a patient’s condition online and give feedback to the patient.
Overview of Innovation:
Virtual Clinic enables patients to manage their conditions more effectively by communicating information about their condition on a daily basis, reducing the need to attend a clinic unless it’s absolutely necessary.

A collaboration between Imobisoft and Birmingham Children’s Hospital, the Virtual Clinic app enables patients to securely communicate information about their condition directly to a clinician while simultaneously updating the Trust’s electronic patient record system.

Currently, patients with long term conditions or requiring post-operative care regularly attend outpatient clinics. The majority of these appointments require no clinical intervention and may require several hours’ travel for what might be a very brief questionnaire-based session with a clinician.
 
If a patient’s condition changes between routine appointments, the app automatically alerts the clinician who can respond immediately and effectively, having been provided with details about the change. An intervention can be made earlier, potentially decreasing the risk of morbidity or the need for more complex interventions or hospital admission.
 
Providing patients with immediate feedback on how their lifestyle or treatment impacts on their symptoms provides immediate personalised education to help them manage their condition.
 
Virtual Clinic is a free app. Users download it onto their phone and enter their unique code to create their profile. Symptoms are recorded via slider bars and radio buttons and sent in real time to the server. Clinicians view the data received, either via an alert when abnormal data is received or by logging onto the web interface to see all data for a particular patient over a period of time. Clinicians can send a message via the web interface to the patient’s phone and run reports on patients, symptoms, etc. from the database.
 
The functionality of the app can be applied to the majority of long term conditions currently managed by regular Outpatient appointments.

1. Adding to the communication channels between patient and consultant at the time that their chronic condition has worsened
2. Improving efficiency in clinical workflows via automatic two-way messaging between the patient’s app and the clinician
3. Providing real time clinical intelligence on a patient’s symptoms to highlight any clinical issues via alerts
4. Patients using Virtual Clinic will not have to attend regular Outpatient appointments if the condition has abated, thereby freeing up Outpatient appointment slots
5. Providing prospective data on patient outcomes, informing future patient management decisions and advances in practice.
 
The implementation of Virtual Clinic is expected to realise improvements in a number of areas:
 
• Redesigned processes associated with monitoring of chronic conditions
• Real-time monitoring when a patient’s condition worsens
• Ability to interact with a patient when their condition initially worsens, rather than at an outpatient appointment when the condition has continued to deteriorate
• Decreased Outpatient appointments
• Decreased A&E attendances and admissions with disease complications
• Improvements in information for reporting purposes via the collection of data regarding a patient’s condition

• Improved outcome data for audit and research
• Improved patient satisfaction.

Virtual Clinic can be customised for various metrics monitored via Outpatient appointments (e.g. incontinence, asthma, diabetes, etc.). Currently, Virtual Clinic is used for patients at Birmingham Children’s Hospital with anorectic malformations and chronic constipation.
